Application Forms

Application for Infant Baptism
Use this form to tell us of you and your child’s details and who the Godparents (sponsors) are. The Baptism register is completed from these details.

Application for Marriage
This is the form from which details are taken and entered into the Marriage Registers, and for the calling of Banns. Information below the signature box is to help identify choices for the wedding ceremony and will be completed later over time.

Application for Banns
This is the form from which details are taken and entered into the Banns Register. Banns must be read or called if you live in a parish but the Church of England place of worship you are getting married in is elsewhere. Information in the boxes with the light grey type is not required for the calling of Banns. A Banns certificate will be issued once they have been read three times, for which a statutory fee is payable.

Completing and returning this form allows us to increase the value of your gifts to All Saints by a quarter (that is for every £1.00 given we are able to claim 25 pence from HM Revenue and Customs out of the income tax or capital gains tax you pay).

The Parish Hall and Old Schoolroom on Sunderland Road (SR6 7UR) are available for hire, please contact the Parish Administrator for availability and charges.

Sacred space is available in the worship area of All Saints off Cleadon Lane (SR6 7UU)
Monday to Friday 9.30am-3.30pm for private prayer.
Donations to the Key2Life Foodbank may also be left here near the font.
